'JAMIE CULLUM LIVE ON THE REC' SIGNED SOUVENIR PROGRAMME FUNDRAISER FOR THE ROYAL UNITED HOSPITAL BATH'S FOREVER FRIENDS APPEAL

Don't miss your chance to own the souvenir programme from Jamie Cullum's 'Live on the Rec' July 31st, 2005, concert that featured Anthony singing with Clare Teal.

For a donation of £25 or more to the Forever Friends Appeal, you will be sent the 27 page programme that contains pictures and features on Anthony, and the cover is autographed by Anthony himself!

You will be helping the Royal United Hospital Bath and become the proud owner of the programme. The Forever Friends Appeal aims to raise £800,000 for a vital second CT Scanner.

How to obtain your copy!

1. Donations of £25 or more can be paid by either credit card, cheques from UK banks, overseas bank draft or UK pounds cash.

2. If paying by credit card, please phone in your card details and name and address to Jo Hones at the RUH.

Please note that calls can only be accepted during UK working hours. There is a five hour ahead time difference between EST and an eight hour ahead time difference between PST.

3. If paying by cheque, bank draft or cash in UK pounds, please send the donation and the name and address for shipment to:

4. Once payments have been received and cleared, the programmes will be shipped out to the address given on a first come first served basis. If the programmes are oversold, and no more copies are available upon receipt of an order that has been delayed in the postal service, the purchasers money will be returned.

6. Please note that only a limited number of signed programmes are available.
